Under the general supervision of the Patient Care Director,the Case Manager / Licensed Professional Therapist (LPC) performs all thenecessary services for hospital clients in need of mental/behavioral healthtreatments. This is an autonomous role that provides and/or coordinates theprovision of case management, assessment, diagnostic impression, referraland/or recommendation for placement, treatment planning, education, dischargeand aftercare planning and all counseling services necessary to identifiedclients. The CM/LPC will provide services for children, adolescents, adults andgeriatric clients in appropriate individual, group, marital, family and/ormulti-family sessions with the understanding that the clients come from allwalks of life including the criminal justice system. The CM/LPC will work with clients in allphases of treatment, i.e.intensive outpatient, partial hospital and inpatientand aftercare.

The CM/LPC will work collaboratively with interdisciplinarystaff and clients internal and external to the organization.Community and public service activities arerequired.Additional duties will includeparticipation in quality improvement, evaluation processes related to themanagement of patient care, and supervision of Interns as the situation occurs.The CM/LPC is available seven days a week and, therefore, is required to worksome evenings, a weekend rotation and an occasional holiday.


Minimum Education : Master's degree required; Graduate of aMaster's Program in Psychology, Social Work, Counseling, or PsychiatricClinical Nurse Specialist required

Required Skills : Requires critical thinking skills, decisivejudgment and the ability to work with minimal supervision. Must be able to workin a stressful environment and take appropriate action.

Basic Life Support(BLS)

Licenses: Currently working towardlicensure in the state of Georgia.

Minimum WorkExperience : Completion ofsupervised, clinical, graduate-level practicum or internship from accreditedschool; OR Three (3) years total related clinical experience which may includethe practicum/internship.
